Application Checklist and Submission Instructions

PRIORITY DUE DATE FOR FALL 2011 ADMISSION :
DECEMBER 1, 2010

Applications submitted to SOPHAS and the UCLA Graduate Division after the December 1st deadline may be considered, but will have reduced opportunities for admission, financial support, and housing..

NOTE: The Fall 2011 application for admission will be available through the Schools of Public Health Application Service (SOPHAS) at www.sophas.org on September 14, 2010.

Application for UCLA will be available September 13, 2010.

We encourage you to submit early.


To apply for admission to the UCLA School of Public Health, applicants MUST complete BOTH of the following application procedures:

    1. Submit an online application to SOPHAS at www.sophas.org
    2. Submit an online application to UCLA at www.gdnet.ucla.edu
    3. Submit appropriate materials to UCLA - School of Public Health

____ Statement of Purpose and Objectives
____ Resume or Curriculum Vitae (CV)
____ 3 Letters of Recommendation - two from former professors and one from an employer. If the applicant has been out of school for over 5 years, three letters from employers are acceptable.
____ One Set of Official Transcripts from each institution you have attended.
____ Official GRE Test Scores - UCLA School of Public Health Code: 4225
____ Official TOEFL or IELTS Scores (for international applicants only)
____ Writing Sample (for doctoral applicants applying to Community Health Sciences or Health Services) 15-20 page limit, download on the CV section
____ SOPHAS Application Fee
(the amount will vary according to the number of applications submitted)

Please note that it can take SOPHAS up to 3-4 weeks to process and send your SOPHAS application to the UCLA - School of Public Health. Incomplete applications will not be reviewed.
