Submitting a Nomination
Thank you for your interest in the 2009 Lester Breslow Alumni Hall of Fame. The deadline for submissions was Monday, December 15, 2008. The UCLA School of Public Health office of Alumni Affairs will host an open call for those who wish to submit nominations for the 36th Annual Lester Breslow Alumni Hall of Fame in the fall of 2009.
Questions about the appropriateness of a particular nomination are welcome and should be directed to Matt Durkan, Director of Development at (310) 825-6464. If there are faculty members who would like to serve on the selection committee this year please contact Megan Aubrey at maubrey@support.ucla.edu or (310) 825-5119.
Nominations must be submitted in English and must include the following:
- Name, Occupational Address, Phone, Fax, and Email of nominator
- Curriculum Vitae and Bibliography of significant publications of nominee
- A Narrative Statement describing the nominee's qualifications for the Award.
- A Proposed Citation - limit to 1 or 2 sentences
