NOTE: The Fall 2009 application for admission will be available through the Schools of Public Health Application Service (SOPHAS) at www.sophas.org on September 7, 2009.
Application for UCLA will be available September 9, 2009.
We encourage you to submit early.
To apply for admission to the UCLA School of Public Health, applicants MUST
complete BOTH of the following application procedures:
1. Submit an online application to SOPHAS at www.sophas.org
2. Submit an online application to UCLA at www.gdnet.ucla.edu
3. Submit appropriate materials to UCLA - School of PUblic Health
____ Statement of Purpose and Objectives
____ Resume or Curriculum Vitae (CV)
____ 3 Letters of Recommendation - two from former professors and one from
an employer. If the applicant has been out of school for over 5 years, three
letters from employers are acceptable.
____ One Set of Official Transcripts
____ Official GRE Test Scores - UCLA School of Public Health Code: 4225
____ Official TOEFL or IELTS Scores (for international applicants only)
____ SOPHAS Application Fee

INCOMPLETE applications (i.e., which lack any of the required items
above) will be processed AFTER completed applications are processed and
forwarded to departments. An applicant may inquire on the status of his/her
