* The original print version of this manual was developed for
a series of HIA training workshops conducted from June 2006 through
April 2007 in Los Angeles, San Francisco and Sacramento. This web
version of the manual was created June 2008 based on the April 2007
version of the printed manual. Support for these trainings and the
