Completed applications must include:
- Official copies of transcripts from every school attended beyond high school. (Send one to SOPHAS directly from the Schools you attended; and one to the MPH/HP office)
- Official report of scores from the GRE. (Send to SOPHAS)
- Three letters of recommendation from former professors and employers. It should be submitted by persons in a position to thoroughly describe the applicant's intellectual and professional capabilities. The letters should be from employment supervisors or academic instructors, not personal friends, family or co-workers. (Send one set to SOPHAS and send another set to the MPH/HP office)
- Resume or Curriculum Vitae (Send to SOPHAS and MPH/HP office)
- A statement of purpose outlining goals and career objectives as they relate to the graduate program. The statement should be no more than three double-spaced pages in length. It should include a concise description of past and present education, employment, and other relevant experience, how these experiences relate to the choice of public health and specially health education and health promotion as a career goal, and a statement describing how a degree in public health and its relevant coursework would be applied to future career goals. - Office report of TOEFL scores for international applicants (Send to SOPHAS and to MPH/HP office)
Applications are now being accepted for Fall 2010. The deadline for application is April 30, 2010 . If you have any questions about the application process, please contact our office.
Keep in mind that the priority deadline for financial aid is early March. You will not be considered for financial aid until you have applied to the program and have been admitted.
