
Application packets for the Residency may be obtained from the Office of Student Affairs at the UCLA School of Public Health.
Physicians interested in both the academic and practicum years should complete the full application packet for admission to the School of Public Health and Graduate Admission at UCLA, in addition to the Residency Program application form. Residents usually begin the program concurrent with the Fall Quarter of the University (usually late September). They may, however, enter the practicum year at other times, with the approval of the Director.
Applicants who have already completed an approved academic year and wish to apply directly for the practicum year should complete the School of Public Health application form, but do not need to complete the Application for Graduate Admissions.
Application deadlines are December 15 (for Fall admission) and June 15 (for Spring admission).
